If you were sent notice regarding the disclosure of your Personal Information, and do not want your name and email address disclosed to the Plaintiffs’ attorneys, then you must opt-out.

If you do not submit an opt-out by December 19, 2025, your personal contact information will be disclosed to the Plaintiffs’ attorneys in this case.

To start your Opt-Out, you must login with your Notice ID and PIN. You can find your Notice ID and PIN on the Notice you received. If you misplaced your notice, please contact the Administrator at (833) 417-4929. Please request your Notice ID & PIN by providing your full name and mailing address.
